Annual Consents

Consents are required on an annual basis. The form below is required as part of annual participation. If the skater is under age 18, parents will have to sign off on each release.

Liability and Injury Release:
I acknowledge that ice skating involves inherent risks, including falls, collisions, and injuries. I voluntarily assume all risks associated with participation in activities sponsored by the Palouse Hills Skating Club, whether as a skater or spectator. I hereby release and hold harmless PHSC from any and all claims, liabilities, or damages arising from injury, loss, or death related to my participation, including, but not limited to equipment failure, surface conditions, or the actions of others.

Physical Health Attestation:
I certify that I am in good health and physically able to participate in the activities of Palouse Hills Skating Club (PHSC).

Media Release:
I, hereby grant Palouse Hills Skating Club the irrevocable right to use my image, likeness, and/or photographs taken on club ice events for any lawful purpose, including publicity, marketing, and distribution in print and digital formats. I understand that my image may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product. I acknowledge that this release is given without expectation of compensation.
